What to Include
Please include the page URL, the statement or figure at issue, the source record if known, and a short description of the requested review. For accessibility reports, include the device, browser, assistive technology, or screen size if it helps explain the barrier.
What Not to Send
Do not send private personal information, confidential records, account numbers, Social Security numbers, medical information, legal demands, or anything that should not be handled through a public-service website inbox.
No Personal Contact Information
DataSplinter does not publish a personal phone number, personal address, or personal email address. Replies, if any, should come from a DataSplinter role-based address.
